Homewatch Caregivers is seeking a Community Outreach Director (Marketer) to join our team.Your primary responsibilities are strengthening the Homewatch brand within the communities we serve.Establishing relationships with key facilities within our service territory.Participating in community and networking events.Creating print ads for local community newspapers and drafting a monthly newsletter column.

In addition, the Community Outreach Director is a key member of our Client Service Team along with a Nursing Supervisor and Scheduler.You will have a limited caseload and be responsible for handling leads (intake calls), meeting with potential clients, and once onboarded acting as the main point of contact for the client, communicating all schedules changes.You will be required to communicate any changes to the client’s schedule or condition in real time to the Client Service Team.In order to succeed in this role, the candidate must possess a can-do attitude, be able to multi-task, and must collaborate effectively with their peers.

Essential duties and responsibilities include the following:

Marketing:

·Develop relationships with targeted referral sources

oTargeted residential communities

§Assisted Living, Independent, and Memory Care communities

oTargeted community organizations

§Community centers

§Temples

§Social Service Organizations

§First Responders

oTargeted Medical Referrals

oSub-acute rehabs

·Attend Networking Events (Chambers,) to establish relationships within the community

·Attend Community Events and Fairs

·Promotional items

·Interview Caregivers of the Month and draft content to be published

·Coordinate all print media advertising

·Responsible for drafting a monthly newsletter column

Client Service-Case Management (limited case load – emphasis on key referral building):

·Schedules in person visit with potential clients to assess their needs (Facilities only)

·Responsible for sending and executing the service agreement

·Build rapport with assigned clients

·Acts as the primary contact for the client/client representative, communicates all schedule changes to the clients in real time

·Resolves all client issues in real time

·Communicates in real time any changes to the client’s schedule or condition to the client service team

·Coordinates Registered Nurse visit and first day of service

·Plays an active role in weekly client service roster meeting

·Assists with being on-call every 5th weekend and once during the week

Administration:

·Weekly activity report

Education & Requirements:

Homecare Experience preferred (or relative field).

Preferred: Licensed Master of Social Work (LSW) required, Licensed Clinical of Social Work (LCSW) At least 2 years of clinical experience, must be fully vaccinated against COVID-19.

Experience & Qualifications:

  • Must be skilled at establishing rapport and providing consultative customer service
  • A winning smile and “can do” attitude
  • Strong oral and written communications skills-articulate, courteous and friendly
  • Attention to detail and ability to work in a fast-paced environment
  • Intermediate computer skills in Microsoft Office products
